POSITION SUMMARY

Utilize peer support to engage and recruit homeless individuals into the Homeless Outreach Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include but are not limited to the following:

  • Identify possible candidates for services in the community's homeless population
  • Develop and maintain relationships with homeless individuals who may need services provided by THP homeless outreach program
  • Model healthy behaviors
  • Relate to clients through shared experience
  • Utilize Homeless Management Information System (HMIS) daily
  • Transport clients to services as needed
  • Express empathy and understanding towards clients in the Homeless Outreach Program
  • Empower choice and independence by allowing individuals to make informed decisions pertaining to their own life
  • Facilitate peer support groups as needed
  • Monitor client progress through frequent meetings with client
  • Maintain clear, concise records documenting services provided as well as client progress
  • Provide crisis intervention services when appropriate
  • Assist clients in developing social and professional support systems
  • Break down barriers to change by meeting individuals wherever they are in their personal process
  • Work in coordination with the Case Manager to assist client in meeting identified goals
  • Conduct weekly, random room checks to ensure cleanliness and proper condition of client rooms
  • Communicate with supervisor and team members on all issues related to clients
  • Attend staff meetings and client case reviews as scheduled
  • Other duties as assigned

QUALIFICATIONS / REQUIREMENTS:

  • High School Diploma or Equivalency
  • Ability to work a flexible schedule
  • Valid Driver's License
  • Strong organization and time-management skills
  • Willingness and ability to travel frequently in community and between facilities
  • Must have or be able to learn and apply basic computer skills, including Microsoft products such as Excel, Word, and Outlook
  • Must be able to achieve CPR certification within 3 months of hire
  • Must be able to navigate stairs and spend majority of workday on your feet (walking, standing)

Indoor environment. The employee is frequently exposed to an office environment with controlled temperatures. The employee is occasionally exposed to outdoor weather conditions including heat, cold, wind, rain, snow, ice, and fog.
